Trempealeau County Health Unveils Online Vaccine Registration Form

(KWNO) The Trempealeau County Health Department has unveiled an online form for eligible residents wishing to get the COVID-19 vaccine.

The form was created after health officials say they’ve received a high volume of calls since announcing members of phase 1B are eligible to get the vaccine. Anyone who’s been able to reach the health department does not need to fill out the online form.

Those who’ve been contacted by their healthcare provider do not need to fill out the form. Health officials say the form will connect residents with a vaccine provider, who will contact them upon completion of the form.
